"First of all, I have to say that the receptionist was very nice and I was able to book an appt. fairly quickly (which I later cancelled)... and here's why:\n\nLong questionnaire with strange and ultra-personal questions! I received an email with a link to a portal to fill out the patient history form. I figured it was a simple health history form, but I began to wonder when it stated: \"Typically, it takes 30 minutes to complete this process.\" And if you don't have time to complete it online, then \"please arrive 45-60 minutes early to complete your questionnaire.\"\n\nThen as I looked at the questions, it was apparent that this was much more than a simple health history questionnaire - some of the questions were beyond personal (none of anyone's business!) and some questions about family members were actually bizarre... and honestly, most of the information they wanted was totally irrelevant to a simple exam. Examples: Ask about using seatbelts, had any type of abuse, if any family members were adopted, or had an accident, does your home cause illness for yourself or your family (what does this mean?). \n\nAnyway, I did not feel comfortable answering these questions and emailed back to cancel my appt. And they did email right back to confirm. If this doesn't bother you, then maybe check out this place. It's just not for me."^^ . "0"^^ .
