I would hire n fire the staff or do a intervention. I have yet to be seen by the doctor, however his assistant seems qualified. I really HATE getting a reminder call minus the time followed by \"if you cannot make it please give us a call you avoid a fee.\"   Why not make sure they understand with initial paperwork and confirm the appt.
