On my last visit, one of the front nurses printed out the wrong patient's summary form and gave it to me. If you want people to be completely careless with your personal medical information, then by all means go here.
When I called to cancel my appointment (which I thought I did that same day when I walked back in there after I realized she handed me the wrong summary) the person on the phone didn't seem to care at all that I was given another person's summary. When I told her why I didn't want to be a patient anymore she just said "okay" no sorry, nothing.
