Allegiant Healthcare of Mesa, formerly known as Symphony, is not a place to send your loved one to for care. Case Management does not include family in weekly meetings or progress reports, nor does the Physical Therapy Department. They take it upon themselves to decide whether or not an outside appointment is necessary or not, without consulting family.- An appointment had been made for my mom that took 6 weeks to get before mom had entered Allegiant. I notified Allegiant of this appointment on 05/31, and received confirmation from Allegiant that same day that they would a schedule transportation for the day of the appointment, 06/20. On 06/21, Mom called advising that transportation never came, and upon inquiry was advised that she was too close to discharge- 8 days away on 06/28- therefore, they decided not to take her. Not only was family not notified to be given the chance to take her, but also the doctor was not called to avoid a 'no show' charge. Myself and my family are highly disappointed in Allegiant Healthcare of Mesa.
