It took over two months to get an appointment with Dr. Boulware for an initial consultation. I arrived early, and was seen late. I saw this doctor for about 30 minutes. He told me what he though the issue was, but did not tell me that it was his official "diagnosis". He handed me a sheet of paper with referrals. He did not help me at all he just made matters worse in the end. I learned of my diagnosis a month later, to my surprise, by the billing department (over the phone!). They told me that BCBC did not cover the diagnosis and that I had to pay up or else they would contact collections. I called BCBS and they told me the Dr. is a professonal and that they have to take his word for it. I called the clinic back and asked to speak with the doctor, and they refused to allow me to speak with him.
Stay away from this doctor if you have any issues. Like I said I was diagnosed and didn't even know it until his billing department contacted me.
Stay away from Arizona Neurological Institute! I believe they inaccurately and prematurely made a diagnosis (after only seeing me for 30 minutes), and now I have been screwed over because of this because my insurance company doesn't pay for this type of "diagnosis".
