  • Terrible place... The following complaint is in reference to my mother's stay at the Life Care Center of Paradise Valley, from March 25th, 2016 to April 14th, 2016. The complaint includes issues with the intake process, Physical Therapy, nursing, and overall lack of communication throughout the facility. As my mother's POA, and son, her overall stay at this facility was stressful. Instead of my attention being able to be placed entirely on spending time with my mother, my time was spent on addressing issues with my mother's needs, created by issues with staff at the facility. My mother was released on a Thursday morning, and Friday night, she was able to walk herself to bed, and put herself into the bed, with no assistance. While I am happy that my mother was able to do this, I also know that this does not mean that she is ready to be completely independent now. I'm also concerned about how much more progress she may have made with better attention, care, and encouragement. The first day that she arrived, she was afraid to walk on her own. She spent 20 days at the facility, and never did any type of walking with a walker outside of PT. Most of her time was spent lying in bed. I am more proud of my mother's resilience, and want to be somewhat independent, more than I am of the facilities work. But I do appreciate what they did to help her. Below, you will find my daily notes that I kept for the duration of her stay. I did my best to include names. The intent for my time spent at the facility was to visit my mother, not to walk around like a health care inspector. But at times I felt like one. I never thought that the notes would become this lengthy. But it became fairly apparent from the start that keeping accurate notes of any and all incidents, and interactions with staff, would become very important. Some things that concerned me were... - Number of times that I spoke with CNA's about her clothes, and who was responsible for washing them - That I never saw my mother with an actual nurse... they only spoke with her to give her her medications. - I never met with a doctor. - The signing of intake documents took a week to be done - That several of my requests for my mother's care were not followed, but I was told that they would be - The lack of communication between the staff, from one shift to the next; I stopped asking nurses for daily information about my Mom because it seemed futile. - The lack of communication between departments; CNA's and nurses not following up where PT left off. If progress is made during PT or OT, it is stifled once back in the care of the nursing staff. - The lack of communication within the PT department; information conveyed to the Head of PT, or other therapists, was not shared amongst them, or it was not carried out accordingly.
