I can't speak to anything else about the health system beyond the ER. The ER is just like every other ER: It's slow, the place is frequented by people who don't actually need emergency services, and if you're there it means something went wrong. It's never a fun visit, but this ER is great with trauma.
Most of the docs and all of the nurses are wonderful people, but a couple docs who are good at saying jerk things to patients. If I'm at the ER, I'm there for urgent medical help, not to be judged or to be on the receiving end of condescending comments. If I'm in poor health, maybe some tough comments are necessary, but I've heard some jerk statements made to healthy yet injured people. That's not cool. Thankfully, as I noted, most of the docs and all of the nurses (that I've encountered anyways) are awesome.
