I heard from Jeff the owner. He wanted me to call and speak with the front desk staff about my experience. I called to speak with the person he recommended, but she was busy, so I left my name and number for her to call me back. She never did, but a week later I received a refund check in the mail for $42. I am glad they tried to make it right, but I've found a great dentist near my home, and he was able to do a filling on a tooth that Tempe Dental said I needed a root canal on. I saved over $1,000 at my new dentist. I think I'll stick with my new dentist.
