Aaron from Bella Patios and John from All Inclusive Concrete agreed that the finished concrete was unacceptable. They offered to apply a textured coating to cover the concrete surface. Job was done a week later and the concrete looks great.
Thank you for taking corrective action.

Update #2 (April 2018, Almost 2 Years Later...):
Well... almost 2 years later and our concrete is still defective! John from All Inclusive Concrete has returned 3 more times since the original pour to attempt to correct the defective concrete. I sincerely appreciate his efforts but the original problem persists. John's crew has tried applying texture once, epoxy twice, and each time they pressure washed the "salt" that keeps surfacing to break up the top coats.
They are doing a great job covering up the problem but these have been temporary "band-aids" and haven't worked. If Aaron from Bella Patios would have taken our original concerns seriously in the first place, this could have been addressed correctly in the beginning. Instead, we've had to deal with the inconvenience, hassle, and wear/tear on our landscaping each time they work on it.
I understand that these things happen, they got a bad batch of concrete or whatever, but this could have been corrected once in the beginning. Our frustration comes from being dismissed by Bella Patios and each time John from All Inclusive Concrete Systems returns, we're reminded of that.
Patience is a virtue and we do appreciate John's willingness to make it right.
I would definitely think twice before hiring Bella Patios, though. Maybe shop around for separate concrete and patio estimates.

Update #3 (June 2018):
The three separate attempts to cover up the concrete failed, completely. This last one lasted a month.
All Inclusive Concrete decided to rip out the original concrete and pour a new slab. They decided removing the patio cover was unnecessary and they jacked it up with braces while they demolished and poured.
Dennis from All Inclusive Concrete assured us everything would turn out okay and be restored to its original condition.
This time the concrete slab doesn't have the "salt" problem, but the slab looks rough. One corner isn't square, the finish looks sloppy against the house, our landscaping was damaged, the patio cover posts are leaning (not level or straight), and the slab edges have blow outs because the lumber they used to form the slab was warped.
No new inspections, no new documentation. Feels wrong!
We've been extremely patient and accommodating as they (concrete company, not Bella Patios) attempt to correct the problem, again and again.
SO DISAPPOINTED! Our backyard doesn't look the same.
This is the most aggravating, inconvenient, and frustrating $5000 we've ever spent.
