  • Notice to the non-hacked! This place either intentionally data mines or is clueless. You are NOT required to give, nor should you EVER, voluntarily fill out your Social Security number on a form that does not expressily require it and explain why. I was stunned to see this on a form for basic bloodwork. A name, an address, and a SS is paradise for an identity thief. Leave the SS blank. Is the minimum wage staff here going to do anything to protect you or that goldmine?! Nooope. Take charge of your personal info and #LEAVEITBLANK
