  • They travel to Chicago, on my last visit (4th visit) each time I was told they "lost" my medical records file. Considering the amount of information they ask for, I, of course, became furious. Once is an accident (hopefully misfiled) twice is incompetent, a third time is ridiculous, and the 4th was a complete disregard to protect PHI. Instead of hearing from someone (manager or owner) with an apology, I heard nothing. I just tried to call and make an appt, and NOW my file is marked that I am no longer a good candidate? Very concerning
