  • When I faxed those papers to my company's underwriter I was told they were filled out wrong I called Dr Muir' s office and left several messages for this office mgr. after the 6th call he called back and when I told him that I might lose my job he asked me to fax everything to him which I did. They are still not filled out correctly. I will go see another doctor as this could cost me my life. I have told my husband to stop referring patients to this Money hungry Quack
