I went for an eye exam and the small office I was taken to had used cotton balls, a wadded up tissue, scraps of paper scattered about the floor. Also observed were two plastic water bottles on the floor, laying sideways under a table. It was obvious to me that cleanliness is not high on their priority list, so I told the assistant that I would not be staying for the examination and left. Oh, and someone should tell front office staff that gum chewing is totally inappropriate during work hours. The office may not be "dirty", but it is unkempt. In a doctor's office? Really? Lack of attention to detail must carry over to other aspects of this practice. Thanks, but no thanks!
