xrays taken, returned to office , where co-pay was collected and then immediatley taken to exam room 11:45am with weight (244# 5'6") and height taken first.saw dr weiner at 1246pm.dr weiner entered sat in dr roll chair next to counter. and asked what was wrong. he did not apologize for wait time.
i stated the incident, being possibly work related? 8-15-16. taking my seat in airline, my left knee gave out and i sat hard down into the middle seat, twisting right knee, unable to move from center seat had to stay in awkward position for entire flight 55min. needed assist to deplane.
i told dr weiner my current job as a shoe fitter and physical wrk requirements. most specifically the shoe fitting bench i need to use to fit customers shoes. i stated that the only thing i was taking for pain was naproxin, but i don't know the mg or dosage i should be taking.
dr weiner asked for me to walk to door of exam room, then asked for me to lie on exam table dr weiner manipulated my left knee. appx 1-2 min. dr sat back down in chair next to counter. , dr weiner stated that according to xrays, which he did not physically have with him, that i had chronic arthritis. he stated that the meniscus was shredded, and what i had was bone on bone, using fists to demo. i was taken aback, because i have not had any issues up to this point. as far as i knew, the twisting of my left knee and knee giving out, while taking my seat in airline, and subsequent twisting of the right, was my first indicator of an issue.
, dr w einer stated, "don't get me wrong, your'e a nice person, but you are grossly overweight, (and something about bmi,) and you most certainly will need knee surgical repair, and you are not a candidate for surgery, because of your weight." dr weiner continued stating that it was my choice, but this was a "wake up call". dr weiner went on several minutes regarding my weight and that it was my choice, but in order to be a surgical candidate, i needed to lose weight.
dr weiner, did not ask me questions on injury or workers comp, or my medical history related to my weight, dr weiner stated that there was no cure for arthritis, and pain management would be the only option adding that he could give me a one-time steroidal injection, and that he would refer me to a pain manage clinic, and that they could also give me a diet.
if dr weiner had listened to me, and had asked clarifing questions regarding my weight he would have learned that i'm on a very healthy diet and exercise regime, water arobics 3x/wk and i swim laps, and at home i pool exercise (30m constant movement), 7 days per wk.i have a very physically demanding job, where i push, pull, lift, climb, and use a shoe fitting bench, i also wear a fit bit and am accountable to a pa/rn support group. i am on my feet, 10 hour days/ 5 days a week. .to assume i have a sedentary life style is ludicrous and presumptuous, while drawing up the injection, dr weiner rattled through the possible side effects, which i didn't have time even process what he was saying. he then asked, "do you live alone?" i stated, no. he replied, sounding surprised that it would be" good to have support'. i have absolutely no idea what that meant. i was excused with a bus card to pain management (which dr w, placed on the counter in the exam room), his parting remark was "good luck to you"at the front i was given the referrals.i was not given any information on arthritis or post care for the injection.sitting in the parking garage, i googled arthritis and was reading about it, when my cell rang, it was dr weiner. he stated that he was reading over my file and that i had noted a possible injury. he stated that to see if the pt and pain management helps, if not, to call back, because i might have a "hair line" fracture. the injection, helped .i went to water aerobics. i googled "steroid" injections, and read to refrain from "formal exercise for 2-4 days." my heart sank, did i just undo the effects of the injection?.using the patient portal , i looked to see if it showed what was in the injection, nothing specific as to exactly what was injected, there was no post care instruction. there was patient education, however it was rheumatoid arthritis, not osteo arthritis., the visit recap stated my age and dx of osteo arthritis. . if my knee giving out is not related to the incident on the airlines, then fine, but dr weiner asked no questions for clarification.i googled osteo arthritis and am unable to find any instance that chronic arthritis can present itself as a collapse of the knee. i learned more from google then from my dr visit.i don't know my prognosis or possible long term care, i don't know what was injected into my knee.,don't know if what i experienced on the airline is related to arthritis or an injury. dr weiner should read his mission statement, and either follow it or rewrite it.
