| - i am currently looking for a private preschool for my daughter. i heard about mccormick preschool from my neighbor.however, when i logged in to arizona department of health services (http://hsapps.azdhs.gov/ls/sod/provider.aspx?type=cc ), i noticed that this preschool was inspected more than once (in 2012, twice in 2013, three times in 2014). they have got deficiencies and a civil penalty...
"licensee agreed to pay $300 in civil penalties for failure to supervise enrolled children at all times; and failure to ensure one staff member had a valid fingerprint clearance card. licensee agreed to return the written documentation of corrections, signed & dated, to the department within 10 days of receipt of the enforcement agreement"

While reviewing the school profile on Arizona Department of Health Services, I found the following important information:
"The purpose of the inspection was to conduct complaint # AZ00122916 investigation on A full inspection was not conducted at this time. The Written Documentation of Corrections was not accepted at the time of the inspection. Ratios observed were: Infants 2:8 1-year-old children 2:11 2-year-old children 2:13 3-year-old children 1:21 4 and 5-year-old children 2:19 There were three staff interviewed during this investigation. There were two staff files reviewed during this investigation. There was one child's file reviewed during this investigation. Others interviewed: The Complainant was interviewed by telephone on April 8, 2014. Documentation observed was: one enrolled child's file, rosters, two staff files, doctor's report on the enrolled child that was hurt, incident report written by a staff member, photographs sent by e-mail from the enrolled child's parent. Upon completion of the complaint investigation, it was determined from observation, interview and documentation that the allegation was substantiated"
I do not think that I will ever enroll my baby in such a place..
