The first thing that I do when I go to a restaurant for the first time is assess the state of the bathroom... A clean bathroom is more likely to represent a clean restaurant. On the way down the hallway, a mop bucket and vacuum cleaner were sitting in open, plain, customer-facing sight. Upon entering the bathroom, the light was off and not automatic, which is alright; the single occupant bathroom had a urinal, which was out of service and covered with a plastic bag, and a standard toilet. The flush handle was inoperable due to a broken chain inside the reservoir attaching to the plug at the bottom of the tank, but there was no indication of such prior to trying the handle.
No offense, but after seeing the lack of attention and care to the back of house, I don't have enough confidence to eat from your kitchen.

Being as I didn't actually try the food, which, I love pho, I wanted to give three stars in your benefit of my doubt, but definitely put attention to the seemingly small details that could indicate larger concerns.
