In June 2015 I went in for the first time. Loved it. Appointment went well. Staff was friendly and professional... however, after weeks of not receiving my card i called Arizona Department of Health and was informed that my card was sent to the address on my paperwork (wrong address). Waited another week and a half for it and got over it. Now this time (May 2016) i went in for a appt a month before my card expired. Again, loved it, staff was great. Doctor welcomed questions and didn't make me feel rushed when answering. Front desk girl, you rocked! A week later i receive am email from Arizona Department of Health stating that there was a problem with my application. Five calls, two voice messages to Southwest Medical Marijuana Evaluation Center and a week and a half later i finally get a responses. My paperwork was resubmitted. A few uneasy days with an expired card went by when i finally received my new card. My eyes were closed in the picture and mouth open because the front desk gal snapped the picture as i was talking to her. I look pretty stupid haha. We are charged a fee to have our application and paperwork process to tried to avoid these things. Third time's a charm?
