rev:text	I was admitted to St. Joseph's hospital on November 18. 2017. After four days I was released. Since that date I have tried too many time to count to get my medical records/test results sent to my doctor's office. My doctor's M.A. is both courteous & efficient. I spoke to her again, today, for the forth time. She assured me she faxed three different request forms to the hospital. We are completely frustrated. While hospitalized I was given an ultrasound on my legs, requested by my PCP, to measure the amount of blood flow through my arteries and veins that supply blood to my legs. Two months later & we still don't have the results. WHY? WHAT WILL IT TAKE...? Shame on medical records department, shame on the Administration Dept. that I called twice with no return call, & shame on the Service Excellence Manager (what an oxymoron!) What now, an email to the hospital's CEO?
