In December 2015 I had been calling the office daily to get a prescription error fixed. Never got a human on the phones so left voicemails. Prescription was finally authorized 15 days later but it was the WRONG prescription so I started calling again daily to have it corrected. A month later I was finally able to pick up my prescription.
Not long after that I received a letter from Dana Forte telling me to take my medical needs elsewhere. Fine!
So now I am not able to obtain medical records. My new doctor has faxed a request 4xs and still no records. I called Forte and was told I could pick them up but it would be 60 cents a page. (which would = hundreds of dollars)
My new doctor advised me that Forte cannot charge me for my records unless I'm requesting duplicate copies or if the records are needed for a legal issue.
Maybe I SHOULD contact an attorney. This is ridicules!
Beware of Joe Efrate...he has no time for your problems. Not knowing me or my condition he actually told me he'd be surprised if I made it home driving without killing someone.
I left in tears.
Hopefully Dr. Hailstone gets out of there before he's dragged down into the swirl of legal issues that will eventually consume Forte Medical Center.
Don't go to this place.
