rev:text	Medical reports, symptoms, and over detail records is atrocious. Different forms indicate different reason for entering ER. In my case, I developed diplopia which is double vision when both eyes are open. One report said my left eye had double vision, another report simply said double vision and a third report indicated Bradycardia. I d o have a pacemaker but was in the ER for sudden diplopia. Furthermore their medical records leave out detail of events i.e. I smoked 52 years ago for less than a year maybe a couple of cigarettes a day. The report said "Former Smoker" is true but anyone reading my report could have interpreted that I just quit and was smoking a couple of packs a day for 30 years. History of meds taken and or stopped lacks any detail of time i.e. how long ago and when stopped or started. General care in ER is generally good except sometimes I think the nurses forget you are there. Biggest problem is that they don't tell you if a specific specialist is on call or on staff. My ER visit was for Diplopia. This hospital does not have an ophthalmologist on call or on staff. I should have been told this in advance. I did have a CAT of the head and was given BP meds because of high BP but I was in worst shape when I was discharged then when I entered. I had nausea and bad headache, Word to the wise. If you need to go to an ER and your visit is specific, make sure the ER has that specific specialist on call or on staff. If not, go to another ER,.
